I give permission for the student named above to participate in martial arts, fitness, self-defense, and related activities provided by Conqueror Hapkido & MMA.

I understand these activities involve physical contact and inherent risks, including falls, collisions, strains, sprains, broken bones, head injury, serious injury, or death. I voluntarily assume all risks associated with participation, whether known or unknown.

To the fullest extent permitted by law, I release and hold harmless Conqueror Hapkido & MMA, its owners, instructors, employees, volunteers, agents, and facility partners from claims arising from participation, except where a release is prohibited by law.

If I cannot be reached in an emergency, I authorize staff to provide first aid, contact emergency services, and obtain reasonably necessary medical care for the student. I understand that I am responsible for medical costs not covered by insurance and that the emergency and medical information I provided must be kept current.

I understand that a concussion is a brain injury and may occur without loss of consciousness. Symptoms can include headache, dizziness, confusion, nausea, balance problems, unusual behavior, vision problems, or sensitivity to light or noise.

I agree that the student must immediately report possible concussion symptoms and will be removed from participation when a concussion or head injury is suspected. The student will not return to activity the same day and may return only after receiving any clearance required by applicable law or program policy from a licensed healthcare provider trained in concussion evaluation and management.

I agree to inform Conqueror Hapkido & MMA of any concussion, head injury, medical restriction, or return-to-activity instruction affecting the student.

The parent or guardian and student agree to show respect toward instructors, staff, volunteers, other students, families, visitors, and the facility; follow instructor directions and all safety rules; use equipment only as directed; and practice self-control and good sportsmanship.

Bullying, harassment, threats, fighting outside authorized instruction, discriminatory or abusive language, unsafe conduct, intentional property damage, and disruptive behavior are not allowed. The parent or guardian will provide accurate health information, report injuries or restrictions promptly, and arrange timely drop-off and pickup.

I understand that unsafe or seriously disruptive conduct may result in removal from an activity or other reasonable disciplinary action consistent with program policy.

I certify that I am the student's parent, legal guardian, or otherwise authorized to complete this registration. I certify that the information I provided is true and complete to the best of my knowledge.

By typing my full legal name and submitting this registration, I intend to sign electronically. I understand that my electronic signature has the same effect as my handwritten signature and applies to each agreement accepted above. The submission date and time will be stored with this registration.
